Where are you located?

We are located in Hunters Creek, Orlando FL at 13564 Village Park Drive Suite 355. We are also able to see you on virtual appointments for patients who are located in Florida.
Do you accept insurance?
We now can see Medicare Part B patients. We accept plain Medicare Part B. As of this moment we are not in network with Medicare HMO plans or Medicare Advantage plans. We can also accept BlueCross Blue Sheild plans however we are considered out of network, so please check to see if you have out of network benefits with your BCBS plan. We are working on getting contracts with insurance companies. . Other insurance that we plan to accept include Oscar, Aetna, Cigna, and United. We will give you an update as soon as we become in network.
Can I submit a claim to my insurance for reimbursement of service?
Yes, absolutely! We encourage you to submit a claim to your insurance to receive reimbursement for the services provided. We will provide you with superbill and other documentation needed for you to submit a claim directly to your insurance. Click here for instructions.
What ages do you see?
We see patients ages 18 and up.
Do you have a patient portal?
Yes, we have a patient portal where you can see your records and communicate with us. You will need to download the Healow app. Click here to start the process.
During a physical, can I discuss new concerns and problems?
A physical is an annual preventative visit which includes a full physical exam, discussing preventative screenings and labs, as well as reviewing immunizations. If you would like to discuss new problems during a preventative visit, we can do that, but it will be an extra fee for established patient visit new concern. This would be a two in one visit. Do you have any policies or rules?
Yes, we do have several policies that we feel are important. To mention a few here, we have a 10 minute late policy, 24 hour cancelation policy, no show policy, and zero tolerance policy. Will you have a lab and imaging center on site?
If I have been Grace's patient in the past, would I be considered a new patient for Summit Primary Care?
Yes. Because this is a new practice, you would need to establish care as a new patient. We will need to input all of your medical history into our system as well as get prior medical records.
