Instructions to submit claims to your insurance

2. Obtain superbill from your visit
We will provide you a detailed superbill after your visit. If you did not receive a superbill and need one, please contact us so we can make sure you receive this. This is not the same as a receipt, this will have details about your diagnosis with codes.
3. Visit your Insurance website
See if your insurance has a medical claims form that needs to be filled out with information about the visit.
4. Submit claim
Follow instructions provided by your insurance and submit all information needed either electronically or by mail. If approved by your insurance, you should get a reimbursement for the service provided.
